      Main duties of the job

      Perform high quality, timely patient assessments and determine a clinical judgement of the patient's condition/needs.Develop, implement and modify individualised treatment programmes, using a person centred and goal focused approach.Monitor and evaluate treatment plans to ensure maximum patient benefit and health outcomes.Ensure appropriate pathways of care and communication via liaison and referral to other agencies as required.Work as an autonomous practitioner with responsibility for your own clinical caseload which will include patients with complex needs.Access regular training, supervision and support from senior staff.

      Within this role, you will have the opportunity to rotate through various specialities, including Acute Stroke, Stroke Rehabilitation, Respiratory/Medicine, General Rehabilitation, Orthopaedics, MSK and Community.

      • Broad range of experience at undergraduate level including respiratory, neurological and musculoskeletal conditions within the NHS.
