- The right candidate needs to be able to work effectively within a collaborative and integrated MDT environment and will contribute to the daily patients care including acute and chronic disease management with a view to promote care planning and proactive patient management initiatives.
- There is scope to work a hybrid role combining remote and face to face working.

- Willingness to take on a lead role or contribute to PCN and service development work
