      What to Expect (Job Responsibilities):
      - Lead the implementation and execution of clinical pharmacy services across multiple practice sites
      - Deliver direct patient care and clinical practices for chronic disease management in collaboration with a medical team
      - Develop treatment algorithms and drug utilization management strategies for the clinical pharmacy program
      - Communicate effectively with the healthcare provider care team to ensure alignment and collaboration
      - Manage questions and issues related to patient therapy while adhering to all regulatory guidelines

      What is Required (Qualifications):
      - Doctorate of Pharmacy from an accredited college or school of pharmacy
      - Current active pharmacist licensure in good standing
      - Completion of a PGY 1 and/or PGY 2 Ambulatory Care residency or at least 4 years of disease state management experience
      - Knowledge of chronic disease states such as diabetes, heart failure, and cardiovascular disease
      - Willingness to travel up to 10% of the time for business purposes

      How to Stand Out (Preferred Qualifications):
      - Bilingual in English and Spanish
      - Multistate licensure and/or willingness to obtain additional state pharmacy licenses
      - Completion of a PGY 1 and PGY 2 Ambulatory Care residency with 5+ years of experience
      - Board Certified (BCACP, BCPS, BC-ADM, CDCES, etc.)
      - At least 1 year of direct or indirect leadership experience

      Responsibilities may include: receiving, scanning, picking, packing, auditing and shipping merchandise to the customer. Tracks all inbound and outbound items through radio frequency (RF) scanning to ensure accountability in inventory. Verifies orders and ensures entry into the inventory management system. Operates a clean and safe work environment, ensuring that all areas of the warehouse are well maintained, kept orderly and free of debris. Maintains quality standards set forth by warehouse management.
