      WE ARE RECRUITING FOR A SENIOR CLINICAL PHARMACIST IN HULL

      A rare and exciting opportunity has arisen for a Senior pharmacist, to join the Medicines Optimisation Team at Haxby Group in Hull.

      Main duties of the job

      We are looking for a highly motivated, flexible and reliable individual who is passionate about patient care. You will have the ability to communicate effectively with clinicians, patients and support staff.

      As a Senior Clinical Pharmacist, you will act within your professional boundaries to provide visible leadership and operational management to the Medicines Optimisation Team. You will assist with the training and performance management of the Medicines Optimisation Team.

      As an autonomous practitioner you are responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making in the management of patients. You will work collaboratively to meet the needs of patients and the practice at all sites or establishments, ensuring the highest standards of care, supporting the delivery of policy and procedures.

      You must work at all times within your own competence and use your own judgement, knowledge, and common sense in your day-to-day activities.

      Clinical Pharmacy responsibilities

      Your duties and responsibilities may include any or all of the items in this job description which may be varied from time to time under the direction of the Director of Prescribing or the Medical Director.

      To adhere to the GPhC code of ethics, observing and applying all professional, ethical and legal obligations.

      Recognise and work within own competencies using own judgement, resourcefulness and common sense.

      Follow agreed protocols, referring to senior staff as appropriate.

      To participate in the education and training for staff, carers and patients regarding medicine management issues.

      To undertake annual clinical medication reviews.

      To assist the wider team in ensuring all patients receive the appropriate drug monitoring required.

      To ensure all major MHRA drug alerts, where appropriate and within your level of competence are acted upon within the practice.

      Follow local and national Infection Control guidelines.

      Demonstrate awareness of statutory and local child protection procedures, including systems of referral.

      Contribute to achievement of the highest possible quality standards such as Quality and Outcomes Framework and other agreed Key Performance Indicator targets.

      To facilitate and manage the development and delivery of a Practice Prescribing and Medicines Management Plan and the delivery of patient services as determined by Practice policy and local and national guidance.

      To support the development of a practice based Medicines Management and Prescribing Team, including delivery of training, in order to maximise cost-effective prescribing and to improve the quality of patient care.

      To promote high quality, cost effective and evidence based use of medicines within the practice.

      To improve understanding of the principles of medicine optimisation.

      To run specialist clinics for patients with long term conditions.

      To ensure that all dispensing errors and near misses are logged in accordance with the companies procedures and to undertake any activity necessary to rectify or prevent errors.

      To liaise with doctors about prescriptions to ensure accuracy and prevent risk.

      To keep up-to-date with current pharmacy practice, new drugs and their uses for the purpose of continuing professional development.

      To promote good practice in line with therapeutic developments.

      To maintain computerised records in accordance with data protection principles to ensure patient confidentiality.

      To counsel and advise patients on the safe and appropriate use of medicines.

      To provide professional advice and guidance on medicines related queries and issues to the wider practice team and patients.

      To advise and assist with all the above as required for Haxby Group and other associated businesses.

      To provide regular support and feedback to the practice on prescribing action plans.

      To update and maintain accurate patient medication records on the practices clinical computer systems including advice given and action taken.

      To advise the Primary Health Care Team on the safe and secure handling of controlled drugs and other medicines, ensuring compliance with medicines legislation.

      Undertake additional tasks as required within your general role as Practice Pharmacist.

      Senior Clinical Pharmacist responsibilities

      You will develop, implement, and adopt standard operating procedures and processes to avoid risk and to enable the team to work more efficiently and proactively, standardising good practice across the business.

      You will have an oversight of the team workload and highlight any areas of concern to the managers.

      You will be responsible for representing the teams at a group level, working with other managers to ensure the requirements of the team are met.

      You will have authority to divert internal and external resources to other locations if the workload requires.

      You will be responsible for liaising with external contactors regarding hours, deployment of staff, work completed, and manage problems with support from the prescribing lead GP.

      Work with the Seniors and Director of Prescribing to ensure sufficient staff of appropriate ability, quality and skill-mix are available to meet current and future service delivery. You will highlight the areas that are impeding the work of medicines optimisation, in particular manpower, staff training and experience, access to technology, and external resource support, and work to resolve these. You will be aware of relevant CQC, QOF, PCN and any other targets that need to be met, their timeframes and what is required to fulfil them. You will ensure enough resources from the practice are in place to achieve these targets.

      Managerial and leadership responsibilities

      You will act as line manager to Clinical Pharmacist(s) in the MOT where agreed on an individual basis. You will promote equality, diversity and inclusion in people management techniques and lead by example. Provide leadership mentorship and support to team members. Role model appropriate behaviours and value and recognise the contribution of all team members and colleagues. Delegate clearly and appropriately, adopting the principles of safe practice and assessing competence. Foster good communications and relationships within the team. Ensure that performance reviews and personal development planning are carried out according to Practice policy for those you manage.
