      About Us

      Our pharmacy team consists of 15 pharmacists and 5 pharmacy technicians, working within a friendly, collaborative, and supportive environment. We have a strong clinical governance framework, with regular clinical supervision, team meetings, and ongoing training led by our senior clinical pharmacists and GP lead.

      Rushcliffe PCN comprises 11 fully engaged GP practices, all of which integrate pharmacy roles effectively within their teams. PartnersHealth, the GP federation for Rushcliffe, is the employer for all additional PCN roles.

      Main duties of the job

      The Role

      As a Senior Clinical Pharmacist, you will take a leadership role within the PCN pharmacy team, contributing to service development and the implementation of best practices in medicines optimisation. Working as an integral part of general practice teams, you will provide expert clinical medicines management across a range of responsibilities, including:

      • Mentoring and supervising junior pharmacists, providing leadership, training, and clinical oversight
      • Conducting monthly 1-to-1 clinical supervision meetings with junior colleagues
      • Undertaking training to become a Designated Prescribing Practitioner (DPP) for pharmacist colleagues and undergraduate students
      • Addressing complex medication-related queries
      • Managing acute prescription requests
      • Supporting hospital discharge prescribing and follow-up
      • Actioning MHRA alerts and conducting targeted medication reviews (telephone and face-to-face)
      • Developing protocols and streamlining prescribing processes to enhance safety and meet CQC requirements
      • Acting as a specialist resource for medicines-related queries, signposting, and liaising with other healthcare professionals

      THE PURPOSE OF THE ROLE

      General Practice is at the heart of the Health and Social Care System, and it is critical to have thriving, sustainable general practices that meet local peoples health needs. The Senior Clinical Pharmacist is instrumental in supporting their own practices clinically and developing and mentoring the more junior pharmacy workforce within Rushcliffe PCN. This is a key role, intended to support the delivery and development of the pharmacist role in primary care and ensure that we continue to provide high quality, accessible and responsive services to both practices and patients.

      KEY RESPONSIBILITIES

      • Works with the Lead Clinical Pharmacist and GP Clinical Lead for pharmacy to deliver the strategy for clinical pharmacy within the PCN.
      • Delivers effective clinical and managerial leadership to allocated members of the pharmacy team.
      • Supports Lead Clinical Pharmacist with the training and clinical supervision of the wider PCN pharmacy team
      • Demonstrates understanding and implementation of appropriate governance.

      • Provides primary support to general practice staff with regards to prescription and medication queries. (helps support the repeat prescriptions system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patient in the GP practice)

      • Provides clinical leadership on medicines optimisation and quality improvement and manages some aspects of the quality and outcomes framework and enhanced services.

      • Ensures that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload.

      Summary of primary duties of the senior PCN pharmacist

      1. Line manager for PCN clinical pharmacy team

      Day to day line management of clinical pharmacy team who have differing competencies and abilities.

      Complete monthly 1-1 reviews with all staff for whom you have line management responsibilities.

      Responsible for 3-month probation review, 6-month probation review and annual appraisals for all staff for whom you have line management responsibilities.

      Responsible for 6 monthly prescribing audits for all Independent Prescribers for whom you have line management responsibilities

      Create development plan for all staff that you have line management responsibility for.

      Follow professional and organisational policies/procedures relating to performance management

      2. Development/training and supervision of PCN clinical pharmacy team.

      Support with recruitment and induction.
