      POSITION SUMMARY:

      Work at home with requirement to reside in or around Newark, NJ

      Shift is Monday-Friday standard business hours.

      No nights, no weekends and no holidays.

      Travel is approximately 50% in Newark, NJ

      As the ACO (Accountable Care Organization) REACH (Realizing Equity Access and Community Health) membership grow s across the country, the ACO REACH Clinical performance team needs resources to ensure high-quality execution of transformation efforts and performance initiatives to drive success of the ACO model. This position will assist in accomplishing the organizational needs by being dedicated directly to our partners.

      • This role is key to facilitating transformation of the care that is delivered to ACO REACH attributed lives, strengthening the patient-centric collaboration between CVS ACO Inc. and our provider networks. The role is provider/clinic facing and is responsible for driving to continuous improvements in quality and patient experience, decreasing the total cost of care and translating to profitable growth.

      • Analyze and report on provider quantitative performance and operational capabilities and to execute a strategic roadmap.

      • This role will have in depth knowledge of each of the practices assigned and consistently collaborate with all stakeholders. The role will also be responsible for supporting clinical performance meetings where applicable.

      • Possess an ability to review, analyze, and address gaps in service excellence, policies, and/or procedures proactively; identify issues and implement solutions.

      • Possess an understanding of the key drivers of value-based care including clinical outcomes, growth, and Profits & Loss statement

      • Collaborating for results across a wide array of team members within ACO Leadership

      • Effective communication and interpersonal skills and an ability to develop relationships across the organizational leadership

      Required Qualifications

      • 3+ years of prior work experience managing operational & quality management KPIs in a healthcare setting

      • 2 +years managed care (MCO) experience

      • 1+ year prior leadership experience

      • 2+ years of analyzing complex data analysis and reporting & deliver presentations to providers

      One of the following licenses is required:

      • Required RN (Registered Nurse) that must hold an unrestricted license in their state of residence, with multi-state/compact privileges and have the ability to be licensed in all non-compact states OR LCSW (Licensed Clinical Social Worker) OR OT (Occupational Therapist) OR P T (Physical Therapist)

      Preferred Qualifications

      • 5+ years of healthcare performance management

      • 2+ years value based care experience

      • Medicare experience

      Education

      Bachelor's required

      Master's Degree preferred

      Pay Range

      The typical pay range for this role is:

      $100,090.25 - $215,600.00

      This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
