      • Monitoring: Supervise qualitative and quantitative data collection, assure fidelity to the ECOP model, evaluate potential problems, and develop solutions in collaboration with program leadership (hub).
      • Stakeholder Relationships: Collaborate with spoke teams to build and maintain relationships with local subject matter experts who form the community advisory boards and key partners. Serve as a point of communication for external contacts.
      • Co-deliver Quarterly Partnership Team Meetings: Organize and participate in partnership team meetings with the spoke teams.
      • Lessons Learned: Leverage qualitative and quantitative program data to identify social and community health needs, gaps, and program impact.

      • Conduct Weekly Supervision Meetings (Spoke): Provide both mentorship and reflective supervision for spoke site Community Coordinators to support professional growth and ensure authenticity of ECOP model adoption.
      • Run Weekly GRA Meetings (Hub): Review status of all spoke sites, facilitate updates on data collection monitoring, solicit team feedback, and assign tasks.
      • Deliver Monthly Full Team Meetings: Plan, organize, and deliver hub-and-spoke team meetings to support implementation and delivery of the ECOP model.
