- Managing a caseload and the provision of mental health care services to people with mental health care need.
- Provide planned care/risk assessment / screening for patients within their care plan and ongoing evaluation of patients' needs and identify suitability for potential discharge in conjunction with the MDT and via caseload management/supervision.
- Initiate and attend patient centred reviews, case conferences and other meetings as required.
- To advise the Responsible Clinician(s) and/or health and social care professional of any change in the patients' conditions and / or circumstances.
- To work within the MDT and to form working partnerships with other care providers.
- To ensure service users/carer(s) are enabled and empowered to make informed decisions about their care and delivery of that care, working with agencies from within and outside the MDT.
- Undertake the supervision of students that will be undertaking placements within the Team.
- Undertake Management and Clinical Supervision for junior colleagues.

- Registered Mental Health or Learning Disability Nurse, Occupational Therapist OR Social Worker with the relevant professional registration

- Ongoing professional development working towards degree level ( if professional registration is following a diploma level course)
- Recognised Leadership/Management training

Job summary
We currently have a substantive Band 6 Community Mental Health Nurse post within CMHT open to applications from Registered Mental Health Nurses. The team is based in West Norfolk covering surrounding areas. We are a recovery focussed, multi-disciplinary team providing an effective, supportive, and professional service for our Services users and Carers.
You will be a qualified professional with experience of supporting people and their families, preferably in the community.You will demonstrate a strong understanding of your role as a Care Coordinator/Assessor and join our dedicated multidisciplinary team working closely with a range of partners to provide appropriate community interventions for those individuals referred to the team.

Main duties of the job
The ideal candidate will have an interest in working within a busy and dynamic Community Mental Health Service. The post holder will undertake the planning/delivery of direct patient care with minimal supervision, ensuring continuity of practice and working within a MDT/multi-agency environment.
Participate in the duty worker rota (Monday to Friday 10:00am to 16:00pm).
