      Due to the innovative transformation of the In-Reach into Care Homes Service we are currently seeking highly skilled, motivated, passionate practitioners with a desire to provide high quality, timely care.

      Recruiting across North and Mid Hants, if you're looking to be to part of service re-design and expansion then we are looking for you. Working in partnership with care homes to provide a behavioural approach to caring for people with distressed behaviour, often due to dementia but you may also care for people with other mental health difficulties, the In-Reach team is keen to support your personal development and welcome you to our innovative service.

      You may already be a B6 or looking for your next step. We are keen to hear from nurses, occupational therapists or any other professionally registered clinician with a desire to work with individuals living in care homes who present with complexity due to their mental health condition.

      Main duties of the job

      The Senior Community Mental Health Practitioner will work as a core member of the Care Home In-Reach pathway supervising junior staff where appropriate.

      Working under the supervision of the Manager/Specialist Practitioner the team member will formulate care and risk management plans in accordance with identified need and ensure that these are recovery focussed and developed in collaboration with the service user and where appropriate their relative and carer.

      Care plans and all interactions will be with a behavioural focused approach.

      Working under the supervision of the Manager/Specialist Practitioner the team member will undertake clinical assessments where appropriate.
