- *Level 1 Registered Nurse.
- *Degree or willingness to undertake.
- *Teaching and Assessing in Practice/Mentorship
- *Post Registration Qualification e.g. rehabilitation, Chronic disease management, Palliative care, History taking, Community Nurse Specialist.
- *Nurse Prescriber.
- *Experience of working with vulnerable adults with complex presentation.
- *Significant range of relevant healthcare. experience in either hospital or community setting including working autonomously.
- *Supervision and training of others e.g. staff, students.
- *Clinical, managerial and leadership skills appropriate to the management of the team. and caseload.
- *Working within a multi-disciplinary team.
- *Post Registration experience.
- *Demonstrate complex nursing skills e.g. medication management, wound care, IV therapy, palliative care management.
- *Demonstrate good time management skills and strategies for organising own workload and that of junior staff day to day, prioritising and delegating appropriately.
- *Demonstrate leadership qualities including an ability to inspire confidence.

Job summary
We are seeking a compassionate and dedicated Senior Community Nurseto join our dynamic UCR team. In this role, you will take responsibility for providing the highest standards of patient care through ongoing assessment, planning, implementation, and evaluation of care programs.
As a Senior Community Nurse, you will work within Nursing and Midwifery guidelines and manage a defined caseload. You will play a pivotal role in meeting the needs of patients and their families, promoting improved health outcomes, and enhancing health and wellbeing. Your work will be guided by values of care, compassion, competence, communication, courage, and commitment. You will ensure that care is comprehensive, patient-centred, culturally sensitive, evidence-based, and promotes independence.
Key Requirements:
*Registered Nurse with valid NMC registration.
*Strong clinical knowledge and expertise in your field of practice.

Provide expert, evidence-based nursing care through assessment, planning, and evaluation of care programs.
