Job Description
- Req#: 523785
- Ensure quality services are provided to each person supported through person-centered planning and maximize opportunities for integration with the community.
- Assist the Supervisor in monitoring and documenting the delivery of all services.
- Implementation of person supported behavioral support plans/strategies and documentation of person supported behaviors.
- Participate in the departmental on-call system including remaining on-call for the work week and participate in the weekend on-call system.
- Assist in the development and monitoring of staffing plans to ensure each person’s health and safety, including the possibility of providing direct care when necessary.

- Bachelor’s degree
- Minimum one (1) year experience working in the field of Human Services
- Clean, valid New York State driver’s license
Senior Community Residential Counselor - OMH Programs
Full-time: Monday-Friday 9a-4:30p with a 30-minute lunch
Salary: $28.84/hour
