      Job summary

      Please note that priority will be given to NCL ICB employees who are affected by change.

      We are recruiting for a Senior Complex Care Practitioner, to join our Barnet team.You will provide line management for the clinical assessment teams and provide staff with expert advice regarding the application of National Guidance and the National Framework for NHS continuing Healthcare.

      The potholder will ensure the team undertakes evidence based health needs assessments for patients to determine eligibility in line with the National Framework, and take overall responsibility for the team's caseload.

      Main duties of the job

      You will lead the recruitment and selection of staff for the team and develop & deliver robust induction, orientation, training and personal development programmes for the staff. The role involves ensuring all staff access supervision, including team learning events, leading the investigation of complaints relating to their team or patients on their caseload, and ensuring the annual appraisal process takes place for all staff within the team. You will support and develop activities for effective teamwork.

      Job responsibilities

      • Deliver comprehensive and co-ordinated specialist clinical guidance, support and leadership to assessors within the Continuing Healthcare Team.
      • Be responsible for the line management of a group of assessors.
      • Provide training and supervision to colleagues regarding NHS Continuing Health care.
      • Organise and manage case load allocation for assessing staff
      • Lead and attend local Appeal and Resolution panels in accordance with National Guidance.
      • Provide clinical input and clinical oversight into the development and review of local policies including SOPs and Personal Health Budgets
      • Participate in monitoring and auditing of care providers.
      • Complete assessments, reviews and provide case management and monitoring of patients in accordance with the National Framework for Continuing Healthcare and Funded Nursing Care. This includes specialist clinical and case management advice and support for thegiven portfolio of patients whose needs may be highly complex, unstable, unpredictable or high-risk needs, liaising effectively with the patient, their family, carers and other professionals.

      Education/Qualifications

      Essential

      • Educated to Masters or equivalent level of experience (see guidance document further details).
      • Evidence of continuing professional development.
      • Extensive knowledge of CHC and associated legal and regulatory frameworks. Evidence of leadership development activities.
      • Current registration with professional body e.g. NMC and hold a clinical qualification e.g. Registered Nurse, Registered Learning Disability Nurse or Registered Mental Health Nurse, AHP.
      • Membership of a professional body

      Experience

      Essential

      • Evidence of managing a team.
      • Evidence of managing a budget.
      • Significant experience in all aspects of CHC, including appeals, LRM, IRPs, retrospectives.
      • Significant experience of managing and supporting others with complex cases and legal aspects pertaining to these types of cases.
      • Experience of working with health and social care systems to deliver effective pathways of care.

      • Previous involvement with efficiency and cost improvement programme.
