Job summary
Senior Nurse Practitioner / Clinical Team Leader - Band 7
Newham Diagnostic Memory Clinic
We are looking for an experienced and enthusiastic Mental Health professional with leadership and supervisory skills to join our Diagnostic Memory Clinic. The post holder should have specialist interest in Dementia and background experience of working with older adults in community mental health setting.
We are currently undergoing an exciting time of transformation, working in partnership with our service users, their carers, Local Authority and voluntary organisations in a drive to continue our goal of providing inclusive and high quality care while developing our service beyond the traditional scope of Diagnosing.
To support us in this process, the successful candidate will be working alongside the Operational Team Lead to provide day-to-day management and clinical leadership to the Team, supporting staff to adapt to the changes and making complex decisions regarding the planning, development and monitoring the care provided.
Main duties of the job
The holder will work alongside the Operational Team Lead as senior practitioner to manage the Diagnostic Memory Clinic within the Community Mental Health Care for Older People of the Newham Locality
Demonstrate ability to manage a service and group of staff, ensuring that standards and quality of care provided are (culturally) sensitive to individual service users needs and have positive impact on their experience and that of their carers'.
Ensure holistic assessment, care and support to people referred to the service for memory assessment, those presenting with co-morbid health care needs.
Effectively coordinate and manage caseload of service users referred and /or waiting for memory assessment, ensuring safety and responsiveness of the Service.
Ensure that service users and their carers are well informed and actively engaged in all stages of the assessment and care delivery process provided by the Service, ensuring that their views are acknowledged and taken into account.
Ensure post-diagnostic feedback and Support, including therapeutic counselling and groups are offered to service users diagnosed with dementia and/or cognitive impairment and their families, ensuring that the support provided are appropriate to meet individual needs.
To manage Nurse-led Titration Clinic run by the Service.
