      Job Description:

      Reporting to the Clinical Director of the Rehabilitative & Transitional Service, the Project Manager will support the planning and implementation of key priorities and initiatives within the Program at Trillium Health Partners. In doing so this position will utilize critical thinking, collaborative approaches and align to best practices to ensure the highest quality, effective and efficient system of care is achieved. This position will lead, plan and implement complex initiatives and service delivery changes in collaboration with Program Leadership, taking ideas from original concept through to final implementation, and champion the development of strong project plans while leading cross-functional multi-disciplinary teams through the execution. The Project Manager is an essential liaison between the Program, internal clinical stakeholder groups, external partners, and the enabling partners to ensure optimal, collaborative and comprehensive planning.

      The successful candidate will help to deliver on key program priorities, including, but not limited to:

      • Overseeing the Program work plan, ensuring alignment between Practice, Quality, Decision Support, Finance and Human Resources
      • Leveraging data, best practices and EPIC capabilities to improve planning priorities & deliverables within the program
      • Partnering with program leadership to identify and implement new service delivery models in alignment with provincial quality standards
      • Supporting the development and coalition of key insights for government advocacy and program planning
      • Monitoring and reporting on progress, problems and proposed solutions as it relates to key program initiatives and escalate issues as appropriate
      • Participating in the creation and delivery of innovative presentations and proposals to all levels of the organization;
      • Demonstrating effectiveness at leading and working with teams
      • Liaising with internal and external stakeholders including regional, provincial and/or private sector partners to ensure the successful completion of program initiatives

      Qualifications:

      • Ideal candidates must have an undergraduate degree in business, life sciences, health informatics or related discipline required.
      • Masters degree in public administration, business administration, or healthcare administration or equivalent experience required.
      • A Project Management Professional (PMP) designation is strongly preferred;
      • Experience implementing and translating performance dashboards and insights to influence action.
      • Thorough understanding of operational needs of inpatient care units, health system flow and operations and demonstrates ability to recognize and remediate gaps
      • Comprehensive experience planning, managing and implementing large scale change management, quality improvement and health system design work
      • Comprehensive knowledge and experience with hospital operations and environments;
      • Superior analytical, critical thinking and problem solving abilities.
      • Demonstrated ability to anticipate needs of senior leaders.
      • Demonstrated effectiveness at leading and working with large/complex teams.
      • Experience organizing and facilitating meetings with multiple stakeholders.
      • Effective interpersonal communications skills and ability to successfully build relationships.
      • Demonstrated ability to develop effective and comprehensive reports and presentations for various audiences, including executives, administrators and clinicians.
      • Excellent organizational, time management and project management skills.
      • Demonstrated ability to multi task and prioritize tasks effectively.
      • Strong understanding of the hospital’s strategic plan.
      • Proficiency in Excel, PowerPoint and Microsoft suite skills.
      • Satisfactory performance and attendance required
