      Job Requirements:

      • Provides direct social work services to patients in their homes
      • Works with the interdisciplinary team in integrating social work services into the plan of care

      Education

      • Master's Degree in Social Work from an accredited School of Social Work

      Required Licensure/Certification

      • Must have Maryland License in Social Work (LCSW-C) with counseling experience
      • Must obtain current CPR certification
      • Must have valid Maryland Driver's License

      Work Experience

      • Home Care experience is preferred
      • Experience with management of adult population
      • Clinical experience in counseling and casework for the terminally, chronically, and acutely ill adult population
