      Job description

      Job responsibilities

Together with your Team, you will deliver high quality nursing care in the community setting out of hours, providing whatever short term input is needed within the scope of practice. In liaison with our Day Team colleagues, we aim to support & maintain our patients in their own homes.

The post holder as part of the Team will provide the care needed whatever that may be. It might be enabling end of life care for the the patient to ensure they achieve their preferred place of death, supporting them, their family & friends to meet that goal. Alternatively, with the Team, you'll be enabling those living with urinary catheters to live independent lives or dealing with whatever urgent health/nursing need may crop up.

The Band 6 shift co-ordinator will manage the clinical needs of the referred patients for that shift. The Service provides all & any urgent nursing care needs which cannot wait until the next day using skilled assessment of need & providing interventions before arranging with Day Teams & other care services for the on-going care of patients as required.

You will need to be confident driver as travelling at night and adverse weather conditions. We provide a pool car for use but you will also need to have your own care available for some shifts.

  • About the company

National Health Service (NHS) is the umbrella term for the publicly-funded healthcare systems of the United Kingdom (UK). The founding principles were that services should be comprehensive, universal and free at the point of delivery—a health service based on clinical need, not ability to pay. Each service provides a comprehensive range of health services, free at the point of use for people ordinarily resident in the United Kingdom apart from dental treatment and optical care.
