      Overview

      Part time 36 hours/week | Variable

      The Service Coordinator provides administrative and logistical support to the multidisciplinary clinical Hospital at Home (HAH) team to ensure operations run smoothly and enhance clinical practice and high- quality patient care. Coordinates communication, patient care services, and nursing supplies to optimize the HAH care delivery model. Works as a team and provides nursing support to achieve the organization's mission of excellent patient care and nursing's vision of setting a new standard of excellence in autonomous and accountable nursing practice committed to patient advocacy and innovative patient care in a climate of trust and collaboration.


      Qualifications

      Minimum Education and Experience Requirements:

      High school graduate or equivalent; AA degree preferred. Experience must include excellent computer skills and applications including Microsoft Office Suite. Excellent interpersonal, conflict management and negotiation skills required. Must function with minimum supervision and be able to exercise sound independent judgment. Must be able to work within a diverse team in a setting where access to and control of confidential information is required.

      Motor Vehicle Operator Designation: Employees in this position: Will not operate vehicles for an assigned business purpose
