      Main duties of the job

      • Deliver high-quality, safe, and effective patient care as part of a collaborative team.
      • Promote a positive working culture by encouraging staff participation in service development and improvement.
      • Manage inbound referrals and respond appropriately to patient and family queries.
      • Update patient records with care plans during MDT meetings.
      • Coordinate follow-ups with the clinical team and liaise with hospital teams and practices regarding patient care.
      • Process patient discharges and ensure all care-related tasks are completed.
      • Report incidents, complaints, and escalations to the Service Manager.
      • Perform additional duties in line with NHS policies and legislative requirements as necessary.

      • Administrative skills and robust record keeping
      • - Ability to effectively communicate and resolve issues over the phone in a professional and courteous manner
      • - Coordinating and liaising with multiple stakeholders or individuals to meet specified outcomes

      Desirable

      • - Experience of working in Health and Social Care, or similar Public Sector roles

      Knowledge and skills

      Essential

      • - Good working knowledge of Microsoft Suite (including Excel, Word, Teams)
      • - General IT set up and systems
      • - Excellent written and verbal communication skills
      • - The ability to plan and organise own workload without direct supervision
      • -Excellent inter-personal skills and the ability to communicate and work effectively with a wide range of people from different backgrounds at all levels of seniority
      • - The ability to be innovative, objective as and logical in resolving problems
      • - The ability to prioritise and balance a range of competing tasks to deliver to agreed timescales
