      What to Expect (Job Responsibilities):
      - Coordinate care needs with participants, natural supports, and healthcare providers
      - Facilitate team meetings to ensure communication among all parties involved at least every six months
      - Document encounters and complete follow-up activities related to participant services
      - Ensure participants maintain eligibility for waiver services and assist with referrals for indicated services
      - Maintain active Medicaid status for participants and assist with benefit management

      What is Required (Qualifications):
      - Experience working with patients with co-occurring mental health, substance abuse, and physical health problems
      - Valid New York State Driver's License
      - Ability to collaborate effectively in a team setting and maintain professional relationships
      - Strong communication skills and ability to engage patients in a therapeutic relationship
      - Educational requirements: Associate’s degree plus 2 years of experience or Bachelor’s degree plus 1 year of experience in case management/service coordination

      How to Stand Out (Preferred Qualifications):
      - Knowledge of community resources, Medicaid services, and available non-Medicaid services
      - Experience with community-based and social support services for individuals with disabilities and/or seniors
      - Familiarity with differential diagnosis of common mental health and/or substance use disorders
      - Strong organizational skills with the ability to exercise independent judgment
