      What to Expect (Job Responsibilities):
      - Ensure efficient transitions of care for patients
      - Perform assessments using clinical skills and CMS criteria
      - Collaborate with healthcare teams to establish optimal discharges
      - Engage with patients, families, or caregivers telephonically or on-site
      - Maintain accurate documentation and adhere to organizational policies

      What is Required (Qualifications):
      - Active, unrestricted registered IN clinical license
      - 3+ years of clinical experience as a Registered Nurse, Physical Therapist, Occupational Therapist, or Speech Therapist
      - Proficient with Outlook, Excel, and Word
      - Access to reliable transportation and high-speed internet
      - Dedicated, distraction-free home office space

      How to Stand Out (Preferred Qualifications):
      - Understanding/knowledge of CMS, Medicare, and Medicaid guidelines/regulations
      - Case Management experience
      - Experience working with geriatric population
      - Exceptional verbal and written communication skills
      - Proven ability to prioritize, plan, and handle multiple tasks/demands simultaneously
