- Practical and procedural knowledge for a range of tasks related to the service area of the policies or procedures relating to at least one other service, e.g. NHS Continuing Health Care; multi-disciplinary assessments.
- Full Driving Licence
- Must have the ability to travel.
- Educated to degree level in a relevant subject or educated to Vocational Level 3 or equivalent significant, relevant, knowledge & experience appropriate to the role.
- Evidence of Continuous Professional Development by producing documentation to meet the Standards of Proficiency for Social Worker registration
- Commitment to work towards the Post Qualifying Award
- Good general education to GCSE level or equivalent.
- Reasonable skills in using PC based applications including Microsoft applications such as Outlook (including calendar), word and excel, on a regular basis.

- Ability to work in partnership with other agencies so that the best outcomes for the service user are achieved.

- Developed communication skills to effectively communicate sometimes complex and/or sensitive information both verbally and in writing with people, as well as to provide feedback to professional colleagues.
- The ability to produce written reports and complete standard recording of service forms and documents, as well as communicating electronically, e.g. email.
- Ability to participate in learning & development by shadowing more experienced Social Care Practitioners and Social Workers.
- The ability to support, advise and give information to assist students on placement in their development, as well as less experienced basic entry band Social Care Practitioners, as appropriate.

Job summary
Social Care Practitioner
37.5 hours per week

A rare opportunity has arisen for a Social Care Practitioner (Band 5) to join Livewell Southwest which is a fully integrated health and social care provider. Experience of working in a health and social care environment is essential. The successful applicants will have a commitment to strengths based and solution focused interventions and use identified models to work with people to achieve person centred outcomes under the Care Act 2014.
This role is working within the Specialist Outreach Recovery Team, supporting, assessing and reviewing individuals within a community setting who have a diagnosis of a complex psychosis. The role offered is working alongside a multi disciplinary team of professionals.
Livewell Southwest is committed to a shared skills model and integrated multi-disciplinary team working to provide the best outcomes for the people of Plymouth.
Band 5 social care practitioners employed within Livewell Southwest work in a range of services including Advice & information Service (AIS), Community assessment and review teams, Care home and Hospital discharge pathways.

Main duties of the job
To provide quality information and advice to people and their carers on the best ways to meet their needs and achieve their stated outcomes and help them to live their lives as independently as possible.
To develop a wide range of skills and knowledge to be able to complete an initial assessment alongside safeguarding enquires.
