      Position Summary:
      The social Determinants of Health Coordinator is responsible for the coordination and alignment of SDOH for people with behavioral health challenges and justice system (BH/CJ) involvement. The SDOH Coordinator will work across the Behavioral Health and Justice Division (BHJD, DBHIDS, the Managing Director's Office of Criminal Justice (MDOCJ), and the justice system to connect programs with available SDOH resources to meet the emerging needs of people with BH/CJ involvement. Given housing's inordinate impact on successful community reintegration, the SDOH Coordinator will pay particular attention to opportunities to expand affordable housing resources for the BHCJ population.
      The SDOH Coordinator will manage the housing and SDOH programs for justice-involved individuals with behavioral health challenges in partnership with the Housing and Homeless Services Unit (TIP). Current housing initiatives include the Returning Citizens Master Lease Initiative, the 100 Day Challenge, Hi-Five, and CHIPPs-funded housing. All housing initiatives involve providing housing subsidies and supportive services to individuals with SMI and co occurring substance use disorders in the criminal justice system. The SDOH Coordinator will ensure flow into and sustainment in the housing initiatives by partnering with BH-CJ programs to identify eligible individuals and coordinating housing and SDOH resources for identified individuals.


      Duties and Responsibilities
      Directs SDOH resources to the BH/CJ population by communicating resource availability BHJD, DBHIDS, and Criminal Justice programs and initiatives.
      Manages the housing programs related to the BH/CJ population, including the Returning Citizens Master Lease Initiative, 100 Day Challenge, CHIPPs-funded housing initiatives, and additional housing initiatives as determined.
      Ensures flow through existing housing initiatives by connecting programs and participants to available housing programs. Identifies target BHJD programs and works with BHJD programs to identify appropriate housing initiative participants. Serves as connector between programs, participants, housing slots, and community providers.
      Supports long term recovery and housing stability by connecting participants to other SDOH resources, e.g., transportation, employment, technology, food.
      Identifies and tracks SDOH resources across DBHIDS and communicates their availability across BHJD. Identifies gaps in resources or access to resources for the BH/CJ population and develops strategies to fill gaps and meet the needs of the populations. Maintains an updated inventory of SDOH resources in use by BHJD programs and initiatives.

      Develops and implements methods for qualifying SDOH resource access and distribution and communicates results to BHJD management.
      Identifies SDOH needs and relevant funding opportunities. Contribute to grant writing and management as needs are identified.
      Collaborates with DBHIDS divisions to align and coordinate SDOH resources to meet the needs of the BH/CJ population.
      Conducts research on the social determents of health and maintains a working knowledge of the latest findings and innovations in the public health field.
      Coordinates BHJD related housing projects, leads in the implementation of new housing initiatives, and manages related project components.
      Participates on the SDOH Committee of DBHIDS and represents BHJD on other committee as needed.


      Education and Experience :
      Bachelor’s degree in social science field required, with Master’s Level coursework preferred. Must have 3 years demonstrated experience in working with the housing, behavioral health and/or criminal justice systems. Valid driver's license and use of personal licensed and insured vehicle during work hours or ability to navigate public transportation to attend sites in person throughout the city.
