Job summary
This is a key role for Beverley PCN offering support to a wide rage of individuals in the Beverley PCN area.
We have 44.000 patients with a wide variety of needs and this would be an exciting and challenging role in being able to identify how best to help and assist them.
Primarily placed within the GP Practices within the PCN, social prescribers enable people to manage and improve their health, well-being and resilience by connecting them in to community services, groups, activities and other pursuits.
Main duties of the job
The role will require management and prioritisation of your own caseload, in accordance with the needs, priorities and any urgent support required by the individual. It is therefore vital that you have a strong awareness and understanding of when it is appropriate or necessary to refer people back to other health professionals/agencies, when the individuals needs is be beyond the scope of this role.
You will support existing groups to be accessible and sustainable and if required, help people to start new community groups, working collaboratively with all local partners. This role therefore requires some multi-agency working as determined by the PCN Leadership Team.
About us
Beverley PCN has been established for 5 years and has a wide variety of staff delivering additional services to aid the patients of the 4 practices within the Beverley PCN group.
The 4 practices are: Greengates Medical Group, Manor Road Surgery, North Beverley Medical Centre and The Old Fire Station Surgery. All 4 practices collaborate to give their patients access to the best services possible.
As the PCN social prescriber, you will be able to directly impact the patients of the PCN and will be supported to become a valued team member of all of the practices.
