      Job summary

      Woolston and Chartwell Partnership are looking for a Social prescriber to join our dynamic forward looking team.

      Main duties of the job

      The successful candidate will have excellent knowledge of the local and national provision and services, be able to work effectively with external organisations and develop care plans, accurately record and report any safeguarding concerns, and sign post patients to support thier physical and mental wellbeing.

      Woolston and Chartwell partnership are two GP surgeries, that merged in October 2023 based in Southampton. With an increasing patient population, we are looking to expand our well established team. We pride ourselves on providing excellent patient centred care in a happy working environment.

      Experience

      Essential

      • A background in person centred practices is essential with good knowledge of equality, diversity and inclusion policies and procedure's. The successful candidate must be an excellent verbal and written communicator with the ability to manage their own diary and organise their time, showing initiative and flexibility. They will have experience of dealing with vulnerable patients and always take a positive and non judgemental approach. They must assess risks and anticipate difficulties and successfully address them.

      Desirable

      • Ideally the successful candidate will have experience working in Primary Care and in a GP practice, be registered with the DBS update service,
      • Proficient using Systmone and possess IT skills with experience using Microsoft teams, outlook etc, Use of a car and be able to travel between sites.
