      Social Services Designee Pay, Benefits and Perks:

      Social Services Designee Essential Functions:

      • Develop specific, measurable, realistic plans and objectives, which enhance social service dail
      • Assure that social service/discharge planning activities are integrated and coordinated within the hospital system by effective communication at all levels
      • Perform psychosocial assessment and reassessment on all residents. Identify and address the psychosocial needs of residents and their famil
      • Perform all aspects of resident care in an environment that optimizes resident safety and reduces the likelihood of medical/health care errors
      • Formulate a teaching plan based upon identified learning needs and evaluate the effectiveness of learning. Include family in teaching as appropriate.
      • Demonstrate knowledge of clinical social work and discharge planning. Interpret and promote residents’ rights and the “Residents’ Bill of Rights”
      • Interact professionally with resident and family and involve resident and family in the formation of the plan of care.
      • Provide individual and group help for residents’ and families at times of adjustment, crisis, or need.
      • Coordinate and direct Social Service/Discharge Planning Department to ensure resident needs are met and facility policy is followed.
      • Provide appropriate resources and support to Social Service/Discharge Planning Staff/Clinical Staff.
      • Be flexible, organized, and function under stressful situations.
      • Maintain a good rapport and working relationship with medical staff, departments, and employees.
      • Manage and operate equipment safely and correctly.
      • Treat patients and their families with respect and dignity.
      • Support and maintain a culture of safety and quality.

      Social Services Designee Skills:

      • Ability to work cooperatively as a member of a team
      • Ability to communicate effectively with residents and their family members, and at all levels of the organization
      • Ability to react decisively and quickly in emergency situations
      • Knowledge of training techniques for clinical staff
      • Ability to maintain confidentiality

      Social Services Designee Requirements:

      • At least 1 year of experience in a Long Term Care setting
      • Active CNA license (preferred)
      • Ability to communicate clearly
      • Good Ethical Practice
      • Re lationship management
