Job Description
- Req#: 10124
- Participate in weekly supervision with assigned Student Supervisor; complete required process recordings and submit prior to weekly supervision. Submit time logs monthly for supervisory sign off.
- Participate in assessments, Care Plans, Billing submission, timely documentation, face to face contacts; Conduct home visits and fieldwork; Conduct outreach activities; Case conference with and obtain necessary records from all primary agencies that are involved with the clients.
- Provide appropriate referrals and ensure follow-up by monitoring the quality of services, verifying, and ensuring client participation; Provide education and supportive counseling to ensure that clients understand and follow up with services to which they are referred.
- Document all services in the required software within 24 hours. Be specific and include comprehensive notes for every service provided.
- Participate in the agency quality improvement and professional development programs, attending internal and external training courses and committees.
- Current MSW or Mental Health Counseling graduate student (1st or 2nd year is acceptable) at an accredited college/university.
- Bilingual
