      Overview

      The Student Navigator is responsible to support one or more care coordinator as a navigator in providing direct follow-up services to meet the client’s needs.

      Responsibilities

      • Participate in weekly supervision with assigned Student Supervisor; complete required process recordings and submit prior to weekly supervision. Submit time logs monthly for supervisory sign off.
      • Participate in assessments, Care Plans, Billing submission, timely documentation, face to face contacts; Conduct home visits and fieldwork; Conduct outreach activities; Case conference with and obtain necessary records from all primary agencies that are involved with the clients.
      • Provide appropriate referrals and ensure follow-up by monitoring the quality of services, verifying, and ensuring client participation; Provide education and supportive counseling to ensure that clients understand and follow up with services to which they are referred.
      • Document all services in the required software within 24 hours. Be specific and include comprehensive notes for every service provided.
      • Participate in the agency quality improvement and professional development programs, attending internal and external training courses and committees.

      Qualifications

      • Current MSW or Mental Health Counseling graduate student (1st or 2nd year is acceptable) at an accredited college/university.
      • Bilingual
      • Interest in working with chronic illnesses, substance abuse, mental illness, and
      • Ability to work collaboratively with other
      • Exceptional written and verbal communication
      • Awareness of and sensitivity to cultural and socioeconomic characteristics of populations
      • Excellent time management and organizational
      • Basic knowledge of computer skills in Microsoft and
