      Job summary

As an Adult Social Care Community Team, we work with a wide range of people with varying presenting needs. This provides excellent opportunities for developing social work skills and knowledge. The team is experienced, supportive, and reflective, consisting of social workers, social care practitioners, occupational therapists, and administrators.

      Main duties of the job

      Assess the needs of service users and carers, understanding their priorities and circumstances, and plan, implement, and evaluate outcomes, working with and for individuals. Identify and deliver genuine person-centred, strengths-based support, connecting service users and their families with suitable community resources and services. Support people to make their own decisions, championing the principles of choice and control to have services organised and risks managed in ways that are right for them. Understand what really matters to our service users, families, and their carers, and work closely with the community and partner agencies, supporting individuals in a positive and responsive way. Work in line with best practice legislation to promote the wellbeing and dignity of service users and take necessary and statutory action in accordance with safeguarding procedures to protect vulnerable adults. Commit to maintaining your professional standards as our service continues to develop and enable the creative use of resources, including assistive technology.

      We Will Expect You To-

      Be highly motivated, passionate about social care, creative, and committed to your own continuous professional development. Be able to work under pressure, be legally literate, and have a good understanding of strengths-based, personalised social work.

      Have strong assessment and analysis skills, as well as the ability to work effectively within a multidisciplinary team.

      Qualifications and Requirements-

      You must be qualified with a social work degree or equivalent and be a registered social worker with Social Work England.

      We welcome applications from Social Work students who are about to graduate but have not yet registered with SWE.

      A full UK driving license and access to a car are often required for client visits.
