Job Description

- Master’s Degree in Social Work
- Previous hospital work experience - Required
- Discharge planning experience - Preferred
- New York State License Masters Social Worker (LMSW)
- Screen certification – Required for Social Workers in In-Patient Units. Effective 1/1/2023, new hires will have 90 days to obtain .
- Broad knowledge of community resources and social work / case management regulatory requirements.
- Knowledge of computer and computer applications – word processing, spreadsheets and clinical application software - Preferred

Description
The Social Worker is accountable for the delivery of social work services including care coordination, planning, concrete and support counseling, entitlements and other community resources regarding chronic disease management. Provides consultation, education, and other services as requested by the clinicians within the Department, in the Hospital or in the community. Provides social work services in accordance with National Association of Social Worker (NASW), Case Management Standards and the Hospital’s mission and goals.
