Job Description
- Req#: 23136
- Demonstrates the ability to effectively collect, interview and document for the MDS assessment.
- Demonstrates maneuverability to efficiently navigate the electronic medical record to enter the sections C, D, and Q of the MDS, Support Services orders and DME needs.
- Makes herself accessible to meet and educate family members/residents on discharge resources and planning needs.
- Maintains up to date effective individualized care plans, admission assessment criteria needs and discharge regulations.
- Licensed Social Worker (LSW) as defined by Sec. 75-53-1 of Mississippi Code of 1972, required
- Basic Life Support
Job Summary:
The Social Worker assists patients and their families with personal and environmental difficulties which predispose illness or interfere with obtaining maximum benefits from health and medical care. Plans, organizes and implements social work services to patients and families. Provides emotional care, advocacy, and maintains effective working relationships with community resources and services. Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served; also demonstrates knowledge of the principles of growth and development and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his/her age specific needs, and to provide the care needed. Assists with completion of MDS process and performing MDS interviews. Implements social service care plans for each resident.

Qualifications:
Education/Skills
A Bachelor’s or Master's degree in social work from an accredited school of social work is required; prefer Master’s Degree; Medical Social Work.
Work Experience:
Prefer at least one year social work experience in a health-related agency or facility.
Certification/Licensure-DUE UPON HIRE
Additional Certification/Licensure - Obtained based on required timeframe below

Ability to organize required tasks, cooperate with others, follow policy/procedure, and relate positively and effectively with patients, families, physicians, and other health professionals, and the general public. Ability to assess clients and initiate and guide others in problem-solving process. Ability to become familiar with community agencies and their policies/procedures. Ability to work independently as well as function effectively as a team member, demonstrate initiative, and function under stress.
