1.0 FTE = 80 hours every 2 weeks
Monday - Friday, Days
One holiday per year
Ensure cooperation and coordination on a regular basis between all key stakeholders for their patients to allow for clear, concise, and timely discharge planning to occur.
Assessment and treatment planning
Discharge planning
Provide patient and family education and support
Documentation and communication - Document all assessments, interventions, and services in the electronic health record.
Starting pay of $26.55 + more for experience!

Bachelor's degree in Social Work or in a related human service field
WI Social Worker license
3-4 years of experience in a medical setting

This highly skilled Social Worker would have additional time to allow for increased collaboration with post-acute care services, county and/or legal resources, physicians, nursing, and other interdisciplinary team members within the organization.
