  Gundersen Health System (becoming Emplify Health) is looking for a full-time Social Worker, Complex Discharge Planner! This highly skilled Social Worker would have additional time to allow for increased collaboration with post-acute care services, county and/or legal resources, physicians, nursing, and other interdisciplinary team members within the organization.

  What you will work:

  • 1.0 FTE = 80 hours every 2 weeks

  • Monday - Friday, Days

  • One holiday per year

  What you will do:

  • Ensure cooperation and coordination on a regular basis between all key stakeholders for their patients to allow for clear, concise, and timely discharge planning to occur.

  • Assessment and treatment planning

  • Discharge planning

  • Provide patient and family education and support

  • Documentation and communication - Document all assessments, interventions, and services in the electronic health record.

  What you will need:

  • Bachelor's degree in Social Work or in a related human service field

  • WI Social Worker license

  • 3-4 years of experience in a medical setting
