Enrich lives including your own as a nursing center Social Worker where you will provide services for our patients/residents, that render the highest practical level of physical, mental, and psychosocial well-being and quality of life.
Position Highlights
*Identify through communication with patients/residents, their family members/significant others, and nursing center staff, patient/resident's strengths, social, emotional, and mental health needs' and then provide, develop, and/or aide them in accessing services to meet their needs.
*Collaborate with the interdisciplinary team to promote and protect resident rights and the well-being of all patients/residents. This includes educating staff on the role of the social services.
*Participate in the development of a written, interdisciplinary plan of care for each patient/resident to address social service needs.
*Hold meetings with patients/residents, families, and significant others as requested and to provide support and information so as to improve quality of life.
*Perform patient/resident discharge planning activities such as family/patient education, nursing center documentation and make referrals to appropriate agencies and suppliers.
*Establish relationships and maintain contact and referral flow with community based agencies/services that benefit our patient/resident population.Who We Are

Qualifications
*Bachelor's degree from accredited school of Social Work or related field is required.
*Must possess any certifications/licensures as required by the state of employment to practice in long-term care.
*One to three years of supervised social work experience in a health care setting, working directly with individuals, is preferred.
*Additional certification such as Geriatric Case Management, Hospice & Palliative Care, Gerontology, Clinical Social Work, Health Care, Nephrology, Mental Health, and/or Substance Abuse is preferred.Pay Target
$25.00 per hour
