      Job summary

      An exciting opportunity to join the award winning Hospital To Home team. The team is highly performing and provides the social work role within Derbyshire Community Health Services (DCHS) Community First service.

      The team is based at the Royal Derby Hospital, however Hospital to Home also work with customers across local Community Hospitals, their own homes and within Perth House assessment centre. Your customer base is people over the age of 18 years who require advice, advocacy and support to maximise their independence, safety and well being. The team works closely with the Integrated discharge team and DCHS to discharge customers using the discharge to assess model.

      Applications are welcomed from candidates with the ability to encourage, support and engage with carers adults both within and recently leaving hospital and people with learning, physical and/or sensory disabilities. A Social Work qualification is essential. There is a strong focus on strength-based practice and ensuring that customers are able to return to the place they call home.

      Derbyshire Community Health Services recently integrated a range of services with Derby City Council that support people to remain at home when their needs change, but also to return home following a hospital admission by receiving rehabilitation and reablement support.

      Main duties of the job

      You will need to be energetic and highly motivated, with excellent communication skills to enable you to work effectively with a wide range of agencies. You should have assessment and support planning skills and experience of person centred approaches. You should have a good knowledge of relevant legislation including making safeguarding personal The Care Act 2014 and Mental Capacity Act 05. You will ensure that you apply consistently high standards.

      The Hospital to Home team is part of a unified health and social care team providing seamless support for local people with the aim of keeping them living as independent as possible, and with good health and mental wellbeing. This includes ensuring that local people access reablement and rehabilitation support that maximises their own skills and abilities, even after a hospital or medical episode.
