- Conducts strengths-based social work assessment and consultation for clients and families of diverse backgrounds
- Assesses family dynamics and function, including strengths, areas of need and high risk situations
- Participate in the development and implementation of child and family counselling services at Holland Bloorview, including developing and delivering education sessions and interventions
- Provide counselling services for caregivers of children and youth with extensive needs with a focus on caregiver mental health and skill building to support the emotion regulation of children with neurodevelopmental or mental health diagnoses
- Design individualized treatment plans collaboratively with the client and/or caregiver based on assessed needs and risk assessment, e.g., suicidality, self-harm, or other safety risks, in conjunction with the other team members of the Extensive Needs Service
- Consult with other clinicians within the circle of care to obtain collateral information, to periodically share progression towards therapeutic goals, to promote effective discharge planning and recommend aftercare services.
- Provide psycho-education sessions to clients and family members, as well as to public audiences and external groups as needed.
- Participate in clinical supervision and training of early career clinicians and learners.
- Liaise with referral sources, community partners and providers, verbally, electronically and/or in writing on a regular basis and documents same.
- Perform duties in accordance with relevant legislation and organizational policies and procedures.
- Participate in review of referrals and consultation with interdisciplinary team.
- Complete detailed and comprehensive documentation including admission and discharge reports and follow-up recommendations in accordance with Holland Bloorview best practises and documentation policies and relevant college requirements.
- Participates in quality improvement initiatives, contributes to the academic agenda through supervision, education and/or research and supports the maintenance of a safe and healthy work environment that advances a culture of client/patient safety through work and daily practices
- Maintain work load measurement and related data collection, as required.

- MSW Degree from an accredited University, Faculty of Social Work
- Registered in good standing with the Ontario College of Social Workers & Social Service Workers
- Demonstrated training and competency in providing evidence-based therapies such as family systems therapy, acceptance and commitment therapy, dialectical behaviour therapy, and other empirically supported modalities and able to apply them in session as appropriate
- A minimum of three years’ experience providing direct clinical services to children, youth and their caregivers in a mental health setting is strongly preferred
- Clinical experience in working with children and youth with neurodevelopmental disabilities, and common co-occurring mental health concerns, such as anxiety, mood conditions, restricted eating and sleep concerns using evidence based modalities and a neurodiversity-affirming approach is required
- Clinical experience in non-violent crisis intervention, suicide risk assessment and de-escalation strategies
- Demonstrated knowledge of relevant legislation (e.g., Regulated Health Care Professions Act, Psychotherapy Act, Mental Health Act, Personal Health Information Protection Act, Health Care Consent Act, Substitute Decisions Act and code of ethics of the College of Social Workers and Social Service Workers
- Excellent written and verbal communication, interpersonal and organizational skills
- Excellent critical thinking, problem-solving, decision-making, time management, planning and evaluation skills
- Demonstrated commitment to the development of professional practice, life-long learning and excellence in client and family integrated care
- Demonstrated understanding of conflict resolution and ability to offer and receive constructive feedback
- Satisfactory criminal reference check and vulnerable sector search required.

This is an exciting opportunity for an innovative Social Worker with experience in child and family counselling to join the Extensive Needs Service, a new program for individuals up to the age of 18, whose needs cannot be met in a single service stream and who require an integrated approach across Mental, Behavioural and Developmental Health. While this is primarily a clinical role, there will be opportunity to work with other members of an interprofessional team to develop and implement the Social Work role with a focus on child and family counselling based on best practices and evidence. As an integral member of the interprofessional team, the Social Worker will provide trauma-informed and compassionate assessment and care plans to children, youth and families with extensive needs such as co-occurring medical, physical and developmental complexities and/or social vulnerabilities. The primary function of this role is to provide child and family counselling for clients receiving care within the Extensive Needs Service.
