      Schedule: Exempt | min. 40 hours per week | 8:30am-5:00pm | Monday-Friday | No Holidays or Weekends!

      What You’ll Do in This Role:

      Be part of something truly special. As a key team member of our Assisted Living program, you’ll help redefine aging by supporting older adults in living with dignity, purpose, and independence. You’ll build trusting relationships, assess individual needs, and coordinate care that honors the whole person. Working alongside an interdisciplinary team, you’ll help craft personalized care plans that empower participants to thrive while safely and confidently aging in place.

      Primary Responsibilities :

      • Serve as a compassionate advocate for older adults by assessing needs, coordinating services, and helping residents thrive while aging in place.
      • Collaborate with the Interdisciplinary Team (IDT) on a person-centered approach.
      • Act independently and take a leadership role in managing resident needs from a social work perspective, with an emphasis on case management and behavioral support.
      • Connect residents and families to community resources, serving as a liaison and advocating for their needs.
      • Contribute to a supportive and collaborative team dynamic, engaging daily with nurses, therapists, and other health professionals to respond to resident changes and adjust care accordingly.
      • Focus on social and behavioral aspects of care, with a strong emphasis on relationship-building and ongoing support.


      What You’ll Need:

      • Current licensure as a Social Worker in the State of Michigan
      • Minimum of one year of experience working in a social work setting preferred.
      • Minimum of one year of experience working with the older adult population preferred.
