      Group Home Social Workers

      assist in the placement of people with Intellectual/Developmental Disabilities (I/DD) into

      care homes, licensed as a California Group Homes, and

      assisting with

      various aspects of the support needs of the people

      in the home.

      The crisis home supports individuals on a short-term basis, twelve to eighteen (12-18) months, and seeks

      to stabilize the them so that they can move into a longer-term living environment as quickly as possible and be successful

      in that new living arrangement. The enhanced behavior home supports individuals to be successful in their living

      arrangement.

      Key/Essential Functions & Responsibilities:

      · Assists in the completion of intake study, needs and services plan and discharge plan for each person in the home, obtaining, developing and recording the information necessary for the completion and ensuring compliance with all applicable regulations.

      · Participates in the transitional planning process, including Individual Education Plan (IEP), Individual Program Plan (IPP), Individual Behavior Support Plan (IBSP) meetings and seeks out community resources that will best support individual needs.

      o Ensures that each individual’s written Needs and Services Plan is regularly reviewed and updated as often as necessary, but at least every six (6) months, to ensure its accuracy, and to document significant occurrences

      that result in changes in the client's physical, mental, psychological, and/or social functioning.

      · Act as a liaison between the home and various parties including family members, authorized representatives and/or advocates of the persons in residence; oversight and social services agencies including CA Department

      of Developmental Services, Department of Social Services – Community Care Licensing Division and Regional Center and other entities such as hospitals, court systems, day programs and/or school districts.

      · Maintains the persons’ services records per CA statute and regulations

      · Provides training for group home staff regarding support needs of the persons moving into the home, needs and services plans, discharge plans as requested by Program Administrator(s) or clinical supervisor.

      · Conducts monthly

      Residential Council Meetings with the people who reside in the home, updating the Administrator, Qualified Behavior Modification Professional (QBMP) and Lead Registered Behavior Technicians (RBT) on the proposed monthly menu and activity schedule.

      · Frequently attend and participate in meetings related to the service and support needs of the persons resident in the home.

      · Attend YAI company-wide meetings, including weekly Social Worker collaboration meeting.

      · Provide monthly recommendations and document using a digital platform.

      · Complete certified trainings such as CPR/FA, CPR and Ukeru.

      · Collaborate with the Registered Nurse (RN), QBMP and Administrator when supporting the people, the

      monthly schedules and appointments.

      · Builds and maintains positive relationships with all levels of staff and the people we support.

      · Assists with the development of new residential programs.

      · Work in partnership with the wider Program Operations team.

      · Performs all other duties as assigned.

      Minimum Qualification Requirements including education, experience, licensure/certification, etc. and essential physical

      capabilities (e.g., lifting, assisting lifting, standing, etc.)

      · Bachelor’s Degree in;

      · o Social work from and accredited program

      · o A field other than social work and at least three (3) years of social work experience under the supervision of a social worker with a Master’s Degree.

      · Ability to work in a stressful environment.

      · Knowledge of California’s Lanterman Act and its related service system, Title 17 and/or Title 22 regulations

      · Highly organized and flexible with ability to manage time and work autonomously while managing multiple priorities and meeting deadlines.

      · Cooperative team player.

      · Exceptional verbal and written communication, interpersonal and presentation/training skills.

      · Project Management.

      · Proficiency in technical systems, such as Behavior Agent, WorkDay, Microsoft Teams, etc.

      · Creative and solution-oriented problem solver with proven influencing skills.

      · Ability to perform designated responsibilities including lifting and/or carrying objects weighing up to 25pounds, reach up to six (6) feet, sit or stand for extended periods of time up to eight (8) hours; under variable temperature conditions (or extreme heat or cold), under variable noise levels, outdoors/indoors, around fumes and/or odor hazards, around dust and/or mite hazards., around chemicals.

      Travel to various regions and programs required.

      Preferred Qualification Requirements (desired requirements beyond MQRs above)

      · Master’s Degree in Social Work or a related field

      · Knowledge and awareness of ID/DD, including knowledge of their unique needs.

      · Familiarity with residential services for people with I/D
