      Job Summary
      To provide Social Services in accordance with acceptable social work practices and hospital policies and procedures. Assists patients and their families with personal and environmental difficulties with predispose illness or interfere with obtaining maximum benefits from medical care.

      Essential Functions and Responsibilities

      • Provide patients and their family with available and appropriate resources, to lead a lifestyle that is as productive as possible.
      • Report child and adult abuse or neglect to the appropriate authorities in accordance with state laws as situations warrant with appropriate documentation.
      • Provide crisis intervention and resources to solve immediate patient and family problems and/or crisis situations with documentation in the medical record.
      • Works as a member of the multidisciplinary team to deliver comprehensive, client-centered, and trauma-informed integrated services.
      • Collaborate with the Ronald McDonald House Charities program team for grant, funding, and quality metric review.
      • Collaborate with the Social Services Departments to communicate current community needs and concerns, as well as update on the latest resources applicable to maternal-child health.
      • Builds understanding of and appreciation for diversity among clients and supports their connection to culturally responsive and culturally specific services as requested.
      • Manage the provision of client support services, including plans for safe care, social service needs, referrals, and community resource linkage.
      • Provide assistance with Medicaid applications for patients.
      • Other duties as assigned.

      Education and Experience

      • Bachelor’s degree in social work from an accredited program.
      • Must have a Bachelors of Social Work (LBSW) License within the applicable state
      • Experience in healthcare, crisis intervention, or social agency at minimum requirement.
      • Requires working with people within the community and coordinating warm hand off for hospitals and partnering agencies.
