Your Opportunity:
The University of Alberta Hospital is one of Canada’s leading clinical, research, and teaching hospitals, and offers a wide range of diagnostic and treatment services, including specialized adult services within renal programs. This position is part of the centralized Rehabilitation and Allied Health Program responsible for acute and ambulatory patients. The Renal Social Worker within Alberta Kidney Care-North provides psychosocial support and counseling to individuals with kidney disease, their families, and caregivers. They assist patients in navigating the complex healthcare system, managing the emotional and practical challenges of living with kidney disease, and accessing necessary resources. They work closely with other healthcare professionals to ensure a holistic approach to patient care. Key Responsibilities include conducting comprehensive psychosocial assessments, adjustment counselling, resource navigation, advocacy, collaboration, education, documentation, and interdisciplinary teamwork. You must be highly motivated, confident, and enthusiastic individual with a positive attitude whose work aligns with AHS Values.
Description:
As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.
Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
Additional Required Qualifications:Minimum 1-2 years’ experience working as an RSW in renal acute care/ambulatory multidisciplinary setting or recent experience working with patients/families facing chronic and/or life-threatening illnesses required. Cultural awareness and sensitivity, experience serving vulnerable populations is essential. The candidate will hold working knowledge of the health care system, community resources, and relevant legislation and decision-making (Personal Directives Act, Mental Health Act). The candidate will have demonstrated strong interpersonal skills to work cooperatively and effectively with teams, patients and their families in a fast-paced environment. Ability to initiate work and make sound professional decisions using critical thinking and problem-solving skills, in a complex multidisciplinary environment. Ability to seek support and guidance from leadership as needed. Good understanding of health promotion and social determinants of health. Strong time management skills are required to prioritize and complete assigned tasks. Current Basic Life Support (BLS-HCP) certification is required. Demonstrated clarity in your communication, organization and consistency of ideas, both verbally and written, including clinical documentation, is required.
Preferred Qualifications:Demonstrated ability to foster collaborative partnerships and relationships. Proficiency with Microsoft office including Word, Excel, Outlook and PowerPoint are preferred. Experience using Connect Care preferred. Master's Degree is an asset. Authorized by ACSW to provide Psychosocial Interventions preferred. Suicide Prevention, Risk Assessment and Management preferred. Non-Violent Crisis intervention preferred.
