      The Bascom Palmer Eye Institute is currently seeking a full time Social Worker, LCSW to work in Miami. The Social Worker, LCSW, provides psychosocial assessments, intervention, and/or treatment and discharge planning to patients and families to assist them in coping with their hospitalization, illness, diagnosis, and/or life situation and meet the continuing needs of patients post hospitalization. The Social Worker, LCSW, administers a broad variety of therapies and is the designated expert, consultant, or source of knowledge within the social work profession.

      Provides psychosocial assessments of patients and families to identify emotional, social, and environmental strengths and problems related to their diagnosis, illness, treatment, and/or life situation. Administers a broad variety of therapies and is the designated expert, consultant, or source of knowledge within the social work profession. Formulates, develops, and implements a plan utilizing appropriate social work interventions. Collaborates with the health care team and involves the patient/family in the development and implementation of plans. Facilitates discharge planning across a multidisciplinary care continuum which includes interacting with consulting physicians, third party payer and other health care providers to facilitate movement of the patient through the system and arrangement of an appropriate discharge plan. Works collaboratively with the multidisciplinary staff to design and implement treatment plans and discharge plans for the patient. Provides education and counseling to patients and families around issues related to adaptation to the patient's illness and/or life situation. May utilize crisis intervention, brief and long-term individual, group, and family therapies. Supervises Master of Social Work (MSW) students. May function as a resource to other staff as appropriate. Provides education to other health care professionals. Participates in or consults on research/education programs.

      1. Advocates for and helps clients gain access to resources while confronting their personal issues.
      2. Counsels and provides crisis intervention for patients, including assessment and treatment of behavioral problems.
      3. Coaches patients on how to manage ongoing health conditions.
      4. Provides therapy to help patients make positive behavior changes.
      5. Assists patients and their families through individual, group, and education conferences to understand, accept, and follow treatment recommendations.
      6. Refers patients and their families to specialized treatment programs in accordance with their needs, and monitors and evaluates their progress.
      7. Collaborates with the other departments to integrate care where needed.
      8. Maintains current and appropriate documentation in patient chart.
      9. Acts as a mandated reporter in suspected child abuse cases.
      10. Adheres to University and unit-level policies and procedures and safeguards University assets.

      CORE QUALIFICATIONS

      Education:

      Master’s Degree in Social Work

      Certification and Licensing:

      Certification in relevant specialty or field

      Experience:

      Minimum 1 year of experience
