Experience working with children and families preferred.
Current LMSW Licensure with the state of Texas.

20Summary:
Seeking a Social Worker to work on a part-time basis to provide case management services to patients and families coping with acute or chronic medical conditions at our High-Risk Asthma clinic. This position will conduct psycho-social assessments, provide social and emotional support, assist with insurance, schedule transportation, and assess the family's need for referrals/linkage to resources. Will work alongside a multi-disciplinary team of healthcare professionals to uphold the Cook Children's promise: Knowing every child's life is sacred, we promise to improve the well-being of every child in our care and our communities.
Hours: 8-hour days (normally Thurs/Fri) and one 1/2 day, days will be M-F but may vary.
