      About the Opportunity
      Primary and Community Health in South Western Sydney Local Heath District are looking for experienced, motivated and passionate Senior Social Workers to support vulnerable families across SWSLHD. The position will be located at either Rosemeadow, Narellan or Hoxton Park Community Health Centres.

      The Primary and Community Health Social Work Department offers a district wide support service to families experiencing complex psychosocial vulnerabilities. We work alongside targeted home visiting nursing programs, allied health clinicians and Paediatricians to support a diverse range of families. Social Workers in Primary and Community Health strive to provide better outcomes for children in South Western Sydney through shared decision making and ongoing liaison and collaboration with key stakeholders in Community, Hospital, other Government Departments and Non Government Organisations. The role is diverse, involving service collaboration, advocacy, case management and consultation. This requires high level understanding of complex family structures, various Health services and the interconnectedness of the service system. The role also requires high-level skill and experience in working both independently and within an MDT.

      As a Senior Social Worker, you will be responsible for providing responsive psychosocial interventions and support to families, mentoring junior Social Work clinicians and completing Quality Improvement projects. You will have opportunity to contribute to the ongoing improvement of clinical practice in Social Work and drive improved client outcomes.

      The Social Work Team supports families from a strengths-based model, providing holistic care with trauma informed, culturally safe interventions. The SWSLHD Social Work Department ensures psychosocial supports and interventions are established in collaboration with the family and are adaptive to reflect their changing needs.

      We are looking for Senior Social Workers with clinical experience of working with women and children in community settings. If you have clinical experience in the areas of domestic violence, child protection, drug and alcohol or mental health and you are child focused, you are strongly encouraged to apply.


      What You'll Be Doing
      This position is responsible for providing specialist Social Work service to consumers and their carers to ensure the provision of optimal health outcomes consistent with SWSLHD policies and procedures, standards, relevant legislation, and the NSW Health Code of Conduct. The position will also participate in professional development, clinical supervision, teaching, quality activities and other relevant departmental and administrative duties.

      SWSLHD Primary & Community Health provides a comprehensive range of community-based services designed to improve and maintain the health and wellbeing of individuals and communities across South Western Sydney.

      These services include prevention, early intervention, assessment, acute and post-acute treatment, health maintenance, and continuing care. The team supports diverse populations, offering specialised programs for children, young people, older adults, and at-risk communities.

      With over 800 dedicated staff, including Aboriginal Health Workers, nurses, doctors, and allied health professionals, the service ensures high-quality care close to where people live.

      1. A Social Work degree which provides eligibility for membership of the Australian Association of Social Workers (AASW).
      2. Extensive post graduate experience working as a Social Worker in relevant clinical areas, including a demonstrated understanding of the health setting, complex health conditions and the impact on wellbeing.
      3. Demonstrated experience in prioritising a challenging and demanding workload; managing stressful situations; and applying advanced reasoning skills and independent professional judgement when dealing with situations of a novel or critical nature
      4. Demonstrated experience in the planning, implementing, leading and evaluating of quality improvement activities involving the application of evidenced based practice within social work
