- Acts as facility liaison with county social workers, community services boards, Medicaid, SSI, Social Security, legal aide, and other services
- Performs assessments of residents
- Bachelor's degree (B.S.) in Social Work or a related field (i.e., Sociology, Psychology, Nursing) from a four-year college or university with experience in a related field and a minimum of five years of related experience and/or training or equivalent combination of education and experience.
- Proficiency in computer skills, including MS Office and Outlook
- Experience working with the elderly in an adult living or long-term care facility with knowledge of geriatric mental health services
- Knowledge of Medicaid/Medicare eligibility requirements

Responsibilities/Accountabilities
We have an amazing opportunity for Social Worker that can:
Required Knowledge, Skills, and Abilities:
§ Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports and business correspondence. Ability to effectively present information and respond to questions from groups of managers, residents, families, staff, and other professionals. Proficient interpersonal relations skills and capability of relating to a variety of personalities and cultures.
§ Understanding of mandated reporting requirements, local and state.
§ Working knowledge of Medicaid and Medicare regulations.
§ Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
Education and Experience:
§ Bachelor’s degree (BSW) in Social Work from an accredited college or university and one to two years related experience and/or training, or equivalent combination of education and experience required. Master’s degree (MSW) from an accredited college or university and one to two years related experience and/or training, or equivalent combination of education experience preferred.
§ Experience working with the elderly in long-term care services with knowledge of geriatric mental health services preferred.
§ Experience working with the mental health population preferred.
Computer knowledge.
Essential Functions:
§ Meets with residents and families to identify significant social, economic, family, or other factors in relation to the resident’s well-being at Birmingham Green.
§ Works in close collaboration with physicians and other healthcare personnel in the resident evaluation and treatment to further their understanding of significant social and emotional factors underlying resident’s health problem.
§ Work with residents, families, and responsible parties to provide support in taking a more proactive role in self-advocacy to improve the quality of life/care of residents
§ Work within an interdisciplinary team to promote and protect resident rights and the psychosocial well-being of each resident.
§ Assist families with financial assistance as required.
§ Completes social histories, discharge plans, assessments, MDS, biopsychosocial and other documentation in a timely manner consistent with the policies, procedures, and applicable government regulations.
§ Provides therapeutic interventions to assist residents, families, and responsible parties in coping with their transition and adjustment to the facility in necessary.
§ Coordinates discharge planning to ensure residents’ safe return to the community.
§ Provides social services in nursing facilities according to the NASW Code of Ethics and Social Work Standards of Practice.
§ Perform other duties as assigned.
Related Duties and Responsibilities:
§ Assists the Admissions Coordinator and Nursing Department in other tasks as needed to accomplish the objectives of the Department and the Birmingham Green.
§ Interdisciplinary Team member.
§ Provide supervision for interns in necessary.
