M Health Fairview has an immediate opening for a Social Work Care Coordinator or Chaplain to support M Health Fairview's University of Minnesota Masonic Children's Hospital and Pediatric clinics. The clinical social worker for pediatric palliative care meets all performance expectations of the Advanced Clinical Social Worker position description. Building on that position, this pediatric palliative clinical social worker provides expert clinical consultation and care to children with serious and complex illnesses and their families following the National Consensus Guidelines for Palliative Care, NHPCO Pediatric Standards, professional standards for palliative care clinical social work and from program leaders to deliver excellent psychosocial care.
ResponsibilitiesThe clinical social worker is a member of the Pediatric Advanced and Complex Care Team (PACCT) working with physicians, advanced practice nurses and unit/program social workers chaplains, and child family life specialists. The PACCT team also collaborates with other pediatric team members from a variety of specialties. The unique aspects of this role are specialized practice in palliative care, working as a clinical social worker in both the hospital and future clinic setting, and being a member of an interdisciplinary consult team that includes palliative care and pain specialists. This team member would oversee bereavement support through coordinating with staff to assure appropriate support and resource allocation for families of children who have died and work to secure and align resources in the form of grants, community resources and donations.
QualificationsRequired
Experience
· New social work graduate with medical experience.
License/Certification/Registration
· Current Minnesota Social Work Licensure
Preferred
Education
· Bachelor’s Degree in social work
Experience
· More than one-year medical social work experience.
OR
Education
· Graduate degree in Theology from approved theological seminary/university, or completion of training in mentored religious tradition as defined by that religious tradition
· Four units of Clinical Pastoral Education (CPE) accredited by Association of Clinical Pastoral Education (ACPE)
Experience
· 1-3 years’ experience as a chaplain
License/Certification/Registration
· Board Certification in one of following organizations: Association of Professional Chaplains (APC), National Association of Catholic Chaplains (NACC), Neshama: Association of Jewish Chaplains (NAJC), or Canadian Association for Spiritual Care/Association Canadienne de Soins Spirituel (CASC/ACSS).
