      Are you passionate about improving the lives of children and families? Join our dedicated Care Navigation team as a Pediatric Primary Care Social Worker, where you’ll play a vital role in helping families thrive through compassionate, comprehensive care.

      What You’ll Do:

      • Collaborate with medical providers to ensure whole-child, family-centered care
      • Help families overcome barriers to care by addressing social determinants of health
      • Serve as a trusted partner and resource to caregivers, connecting them with vital services and support

      Required Education:

      • Master’s Degree in Social Work (MSW)

      Skills & Experience:

      • Strong problem-solving, critical thinking, and communication skills

      • Ability to work independently with attention to detail and high computer literacy

      • Proven ability to build rapport with patients, families, and interdisciplinary teams

      Licensure:

      • Active LISW license
