- Must be able to provide social services in a hospital setting in the state of Arizona.
- Must be a Licensed Clinical Social Worker.
- Must obtain CPR and Handle with Care within 90 days of hire.

Position Summary:
Social work staff to support and enhance the mission of Canyon Vista Medical Center in providing excellence in healthcare to patients and the community. Social work staff to provide guidance for the assessment and reassessment of the psychosocial needs of the patients, families and caregivers. Social Worker is dedicated to community awareness that promotes the goals of the hospital through excellence, practice and leadership. Social work staff to assist with complex discharges to support the Behavioral Health department and other discharge needs. Social worker facilitates safe discharge from emergency care and transitions patients into the continuum of care arena as needed. Coordinates and assists other social workers in the BH Department. Complies with legal, hospital and departmental policies, procedures and position requirements.
