- Providing specialist occupational therapy care and holistic assessment of patients in the community as an autonomous practitioner, interpret results appropriately, use clinical reasoning skills and knowledge of evidence-based practice, in relation to the individual's clinical and psycho-social issues to determine treatment options, recommending the best course of action.
- To be responsible for a clinical community-based caseload working within multi-disciplinary teams, sharing care appropriately ensuring clients and carers receive optimum level of therapy and holistic care within available resources.
- You will identify and manage risk through comprehensive home environmental assessments and provision of appropriate equipment/aids and environment modification to reduce readmission to hospital and facilitate rehabilitation and/or manage disability
- A comprehensive induction into the community service followed by a local induction to introduce you to the role

- Degree or diploma in occupational therapy
- Registered with the Health Care Professions Council
- Up to date relevant knowledge through CPD
- Hold relevant right to work in the UK documentation

- Membership of Royal College of Occupational Therapy and / or relevant special interest groups
- Attendance at Clinical Educator Course
- Appropriate post-qualifying NHS experience in a range of areas e.g. care of the elderly, adult community rehabilitation, falls, experience with a variety of medical conditions/pathologies
- Appreciation of constraints to working in the community
- A clear understanding of current NHS issues, professional standards, CPD, clinical governance and experience of audit
- Ability to assess patient conditions, establish the patient's problem, formulate and implement a treatment plan
- Has some knowledge of the Mental Health Act and other legislation
- Experience of MDT working and understand their roles
- Experience of supervising juniors/assistant/students
- Experience in research / literature reviews
- Clinical Teaching
- Experience of using electronic clinical system
- Using EMIS clinical system
- Previous community experience within rehab
- 2 years post graduate experience
- Effective written and spoken communication skills
- Proficient with basic computer skills (excel, word)
- Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010)
- Degree or diploma in occupational therapy

Specialist Band 6 Occupational therapist, Discharge to Assess, Central London Community Healthcare NHS Trust (CLCH) - Wandsworth.
Are you a Band 6 occupational therapist or experienced Band 5 looking for an exciting, new challenge and a chance to take on greater responsibilities?
We are looking for a specialist, dynamic and motivated Occupational therapist with excellent clinical skills and a positive attitude and commitment to providing excellent holistic community-based care. The person will join our group of AHP staff in a permanent role and supporting an MDT community therapy service.
Discharge to assess (D2A) is a 7 day a week community assessment and rehabilitation team in Wandsworth. It provides therapy services to specific types of patients upon discharge from hospital. These patients are often more urgent and/or complex upon discharge to the community. The team supports the discharge to ensure readmission is avoided. This involves risk management, disability management and rehabilitation. The team sees a wide variety of conditions/pathologies and provides therapy in a holistic way using a wide variety of knowledge and skills. The multidisciplinary team consists of Physiotherapists, Occupational therapist and rehabilitation assistants.
