What to Expect (Job Responsibilities):
- Review complex referral requests and evaluate appropriate specialists for patients
- Work with patients and providers to understand services being requested
- Coordinate care within Atrius Health and with external partners
- Ensure clinical handoffs are safe and appropriate
- Interview patients as needed to obtain full understanding of information requested
What is Required (Qualifications):
- High School diploma or equivalency certificate required
- Minimum of three years in a clinical or customer service setting preferred
- Strong technical proficiency in Microsoft, scheduling software, and electronic medical records systems
- Strong problem-solving and complex patient management skills preferred
- Relevant experience in the managed care environment preferred
