      Hampshire and Isle of Wight Health Care NHS Foundation Trust has an exciting opportunity for a Band 5 Speech and Language Therapist to join our dynamic, friendly and supportive Adult Community SLT team to work with adults with communication and swallowing difficulties.

      We are looking for an enthusiastic Speech and Language Therapist with post-qualification dysphagia training and dysphagia experience to provide a comprehensive, specialist clinical service to clients with communication and swallowing difficulties. The post-holder will work with clients with a range of conditions including stroke, progressive neurological disorders, dementia, and learning disabilities. You will manage your caseload independently with support from peer and senior Speech and Language Therapy colleagues.

      Our Speech and Language Therapy team covers a wide geographical area across Portsmouth City, Fareham & Gosport and South-East Hampshire, in both outpatient and domiciliary settings. Clinical care pathways are in place to support a client's journey through the service, led by highly specialist SLTs with active contribution by all team members.

      Main duties of the job

      The SLT department provides a service to people aged 16 years and over with an acquired communication, voice or swallowing difficulty who live in the Portsmouth City, Fareham and Gosport and South-East Hampshire areas. Interventions are delivered face to face, in groups, via the telephone and over video platforms.

      The SLT service has multiple clinical pathways including AAC, aphasia, dysphagia, dysfluency, dementia/ Older People's Mental Health, progressive neurological conditions, and voice. Each pathway is led by a highly specialist SLT.

      Robust referral, triage and prioritisation processes are in place, supporting timely, effective and evidence- based input to patients. The priority is to provide equitable access to the SLT service across all geographical areas and clinical care pathways.

      The SLT service proactively supports patient self-management and education, providing advice, guidance and signposting and supporting discharge.
