      Overview:


      Schedule: Weekday shifts 4p-12:30a, every 4th weekend shift will be 7p-7:30a, holiday rotation commitment, 20 hours per week


      Job Description Summary:


      Coordinates patient admissions, discharges, and transfers, acting as a liaison between healthcare providers to ensure seamless patient care and appropriate placement. Manages patient triage calls and facilitates consultations. Practices professional nursing and promotes patient advocacy in accordance with the ANA Code of Ethics for Nurses and maintains a current Ohio Nursing License.


      Job Description:


      Essential Functions:

      • Coordinates the process of admitting, discharging, and transferring patients, ensuring adherence to hospital guidelines and protocols.
      • Acts as a liaison between various healthcare providers, including medical staff, patient care services, and external providers, to ensure seamless patient care and appropriate placement.
      • Manages the Primary Care Phone Triage Line, assessing the urgency of medical concerns and determining the appropriate level of care and response needed.
      • Facilitates pediatric specialist consultations, including telemedicine consults.
      • Participates in professional development and quality improvement projects to maintain and enhance professional standards.
      • Ensures accurate documentation for patient admissions, transfers, and consultations, and communicates pertinent information to relevant personnel.

      Education Requirement:

      Bachelor’s Degree in Nursing from and approved school of nursing, required.

      Licensure Requirement:

      Licensed as a Registered Nurse in the State of Ohio.

      Certifications:

      • Current CPR certification required.
      • Must be willing to obtain and maintain NRP, PALS, ACLS, APHON or other necessary certification as determined by your manager based on business unit.

      Skills:

      • Excellent verbal and written communication and interpersonal skills.
      • Effective computer or main frame systems skills.
      • Excellent organizational and critical thinking skills.

      Experience:

      • Three years of clinical experience, required.
      • Pediatric clinical experience, required.
