      Job summary

      An exciting opportunity has arisen to join our in-reach rehab beds team and community Therapy Hub as a static B5 Occupational Therapist.

      We are looking for highly motivated applicants with excellent communication skills and a drive for clinical excellence. The post offers the opportunity to work in both:

      • In-reach Rehabilitation Services - covering step down beds in set care homes
      • Community Therapy Hubs - providing rehabilitation in people's homes, with teams based at either Brighton General Hospital or Hove Polyclinic.

      Main duties of the job

      The post will start with supporting the rehab beds. You will carry a supervised caseload, for which you will provide client centered assessment, treatment planning, goal setting, discharge planning and evaluation.

      With the therapy hub you will carry out client centered assessment, treatment planning, goal setting, and evaluation within peoples own homes.

      You will be provided with a robust support system of clinical supervision, preceptorship, a personal development plan, and opportunities for training.

      You will work as a key member of a multi-professional team including support staff.

      You will will write thorough and timely documentation.

      You will contribute to the smooth running of your service.

      To manage a caseload of patients with a variety of conditions.

      To formulate and deliver an individual treatment programme based on a sound knowledge of evidence-based practice and treatment options using clinical assessment and reasoning skills.

      To treat all patients with respect and courtesy, maintaining dignity and respecting individual needs and differences, both physical and cultural.

      To complete the SCFT Preceptorship programme through regular supervision and professional support (staff in their first year post-qualification).

      To be involved in the support of Band 2, 3 and 4 staff as well as student education.

      To take part in and contribute to in-service training and keep your own personal development plans up to date.
