      STREET MEDICINE VAN COORDINATORS

      (Full Time/35 hours per week)

      Janian Medical Care, P.C., an affiliate of Center for Urban Community Services (CUCS) seeks a Street Medicine Van Coordinator to join our expanding Division of Primary Care Services, who will perform daily driving/van coordination and management tasks with the option of also assisting with Medical Assistant tasks.

      Janian Medical Care provides primary medical care and psychiatric care to homeless and formerly homeless New Yorkers at community-based programs including supportive and transitional housing, shelters, and with street outreach teams. Long recognized as a leader in the development and delivery of psychiatric care, Janian Medical Care continues to expand to provide comprehensive primary care services to people staying on the street, in Manhattan, Brooklyn and Queens. The street medicine program includes two medical vans to bring primary care and nursing services to individuals experiencing street homelessness who have been identified by street outreach teams. We are seeking a dynamic, collaborative Van Coordinator, dedicated to supporting Janian's work in providing street medicine to some of NYC's most medically complex and vulnerable individuals.

      Janian invites applicants for the Street Medicine Van Coordinator position to work alongside the primary care providers and nurses on Janian’s Street Medicine Teams in Manhattan, coordinating all aspects of the program’s mobile Medical Van. This includes driving the van between teams and street locations, coordinating van schedule, including parking in designated garages, and managing medical supply inventories on medical van and at street medicine offices. If interested and have received training, the Street Medicine Van Coordinators also have the option to work directly with patients.

      The Street Medicine Van Coordinator will work in collaboration with both Janian Street Medicine providers and RNs as well as the outreach programs’ social workers and case managers. Program services include: street outreach, housing placement, comprehensive health, mental health, and case management services.

      The Street Medicine Van Coordinator position is well suited to someone with a background in driving and outreach or case management, who can work closely with multiple teams and is interested in/has a background in healthcare and serving underserved populations, especially street homeless clients. The position is, also, well suited to someone planning to go to medical, physician assistant, or nursing school who would like to experience working in a medical setting alongside providers during a gap year.

      Responsibilities :

      The Street Medicine Van Coordinator will perform administrative duties under the direction of a team of providers. If interested, the ideal candidate will also have the ability to participate in certain clinical activities.

      Duties include the following:

      • Drive Medical Van
      • Coordinate Medical Van parking
      • Cleaning and basic maintenance of van and van equipment
      • Coordinate upkeep and repair of van and van equipment when necessary
      • Coordinate Medical Van schedule between teams and locations
      • Evaluate how the vans are used to bring suggestions for improvement
      • Assess efficiency and suggest new uses to maximize health benefit to the population
      • Pick up and delivery of medical supplies to teams and patients, delivery of samples to laboratories
      • Medical supply and equipment maintenance including quality control testing and inventory management
      • Communicate with Street Medicine providers about their use of Medical Van on a day-by-day basis
      • Administrative tasks may include: basic intake and collection of client information and insurance coverage verifications

      Additional clinical training and involvement is available (but not required). Optional clinical duties might include the following:

      • Taking and recording vital signs
      • Preparing patients for examination and performing basic screenings
      • Laboratory services including phlebotomy
      • EKG exams
      • Point-of-care testing as directed by the provider

      The Street Medicine Van Coordinator role with Janian is dynamic, and additional tasks and responsibilities may be requested.

      Minimum Qualifications:

      • High school graduate or GED
      • Valid NY State (or tri-state) Driver’s License: Experienced driver with clean driving record
      • Comfort driving in New York City and familiarity with geography of Manhattan, Brooklyn, and Queens or comfort using GPS
      • Must be willing to travel during the course of the work week to our various locations to work with assigned providers
      • Must be highly organized, flexible in workstyle and and have strong problem-solving skills
      • Must work well both on a team and independently
      • Strong written and verbal communication skills
      • Proficiency in the use of Microsoft Word, Excel, and Outlook
      • Proficiency in the use of eCW preferred, but not required
      • Familiarity with medical terminology and electronic medical record keeping preferred, but not required
      • CPR certification is a plus, but not required
      • Associates Degree/Bachelors Degree with case-management experience or strong interest and aptitude for working with people experiencing homelessness preferred

      * Bilingual-Spanish speaking applicants encouraged to apply. This position is 9am-5pm Monday through Friday with some scheduling flexibility required depending on the day’s schedule.
